Lund-Mackay Score

  • The Lund-Mackay score is a commonly used way of performing radiologic staging of chronic rhinosinusitis.1
  • While the Lund-Mackay score does not correlate with subjective symptom scores, it does correlate well with the extent of surgery required, the degree of post-operative symptomatic improvement, and the likelihood of requiring revision surgery.2
  • The score varies from 0-24, and assigns points (0-2) based on involvement of 6 areas of the paranasal sinuses on both the left and right hand side:
    1. Ostiomeatal Complex (0 or 2)
    2. Anterior Ethmoids (0, 1, 2)
    3. Posterior Ethmoids (0, 1, 2)
    4. Sphenoid Sinus (0, 1, 2)
    5. Frontal Sinus (0, 1, 2)
    6. Maxillary Sinus (0, 1, 2)

  • The score for each location is assigned as:
    • 0: Not opacified
    • 1: Partially opacified
    • 2: Completely opacified
  • The only exception is the ostiomeatal complex, which is assigned a score of 0 if not obstructed, versus 2 if obstructed (you can’t have a score of 1 for the OMC).
