Barany Criteria for Meniere’s Disease

  • Supplants the prior 1985 AAO criteria for Menière’s disease.
  • Divided into definite MD and probable MD1:
    • Definite Menière’s Disease
      1. Two or more episodes of spontaneous vertigo, each lasting 20 minutes to 12 hours.
      2. Fluctuating aural symptoms (hearing changes, tinnitus, aural fullness) before, during, or after an episode of vertigo.
      3. Audiometrically documented low to medium frequency SNHL in one ear in at least one occasion, starting before, during, or after an episode of vertigo.
      4. Not better explained by another vestibular diagnosis
    • Probable Menière’s Disease
      1. Two or more episodes of vertigo or dizziness, each lasting 20 minutes to 24 hours.
      2. Fluctuating aural symptoms (hearing changes, tinnitus, aural fullness) in the affected ear.
      3. Not better accounted for by another vestibular diagnosis.
  • Removes the prior classification of Certain Menière’s Disease from the 1985 AAO Criteria.
    • This had previously required meeting the criteria for Definite Menière’s disease, as well as histopathologic confirmation (which is not feasible in living patients).
